Passenger Van Certification/Request

The following steps are required in order to request the use of a University van. Please see the Criteria and Guidelines for Use of University Vehicles further below.

1. Request for Driver Information

Pittsburgh's Office of Fleet Services requires all individuals wanting to use a university vehicle to complete the “Request for Driver Information” (DL-503) form prior to driving our vans. This form must be completed and emailed to fleetservices@bas.pitt.edu prior to driving a University van, along with a copy of your driver’s license and Pitt ID card prior to driving a University van. Please include the following information in your email:

  • Department Name
  • Vehicle #(s) you are driving
  • Immediate Supervisor Name & Email
  • Department Administrator Name & Email

Drivers are required to send this information to the Office of Fleet Services every year.

Once this information is submitted, you will receive an email from the Office of Fleet Services when you have been approved. Please forward a copy of that email to upgss@pitt.edu for our files. You will then be able to reserve and drive the van(s) for University purposes.

2. Mobile Device Guidelines

All drivers must also complete the "Mobile Device Guidelines." Please forward this form to your immediate supervisor for their files. Do not send this form to the Office of Fleet Services.

3. Van Request

When reserving a vehicle, remember that vans must be requested at least ten business days in advance by completing the Van Request Form.  A Journal Transfer will be sent to the account administrator for approval and payment.  The daily charge for using a University van is $50 per van.

Criteria and Guidelines for Use of University Vehicles

The following criteria must be met to obtain approval for anyone to drive a University vehicle:

  • Must have a valid Pitt ID card.
  • Must be enrolled/employed at the University.
  • Must have a valid driver’s license.
  • Must be 21 years of age and have had a valid driver’s license for two years, not at fault for more than one accident within the past three years and possess no more than two traffic violations within the past three years to drive a vehicle that seats 7 or less.
  • Must complete the Van Request Form that includes dates destination, and names of all passengers.
  • Must complete and sign the Drivers Release sections (C & E) of the Department of Motor Vehicles (DMV) Request for Driver Information form (DL-503).  Information obtained from the DMV, including actions and violations, will be reviewed and may be used to disqualify individuals from driving a University vehicle. These forms, along with a copy of a valid Pitt ID and Driver’s license must be emailed to fleetservices@bas.pitt.edu.
  • The individual authorizing the vehicle usage will be considered the responsible party.
  • For trips over 250 miles one way:
    • Two approved drivers are required.
    • Drivers must rotate every three hours.
    • No more than 10 hours of driving can be completed during any one day.
    • Driving between 11:00 pm and 6:00 am is prohibited.
  • Driver Safety Training Course - new and revised course that the Pittsburgh campus is implementing.

Anyone granted permission to use University vehicles must agree to the following rules and regulations:

  • Use vehicles for the official stated purpose only.
  • Operate University vehicle in accordance with University rules and regulations.
  • Observe all applicable traffic laws, ordinances and regulations.
  • Use safe driving practices at all times.
  • Use seat belts at all times.
  • Driver assumes all responsibility for fines or traffic violations.
  • Do not permit unauthorized individuals to drive or ride in the vehicle.
  • Ensure that all doors and windows are locked when vehicle is not in use.
  • Inspect vehicle before leaving and after returning to the point of pick-up.
  • No smoking in University vehicles.
  • Clean the vehicle of all trash.
  • Using a cell phone or eating is prohibited while driving the van.
  • No animals are allowed in the van, with the exception of service animals.
  • Van(s) will include gas prior to rental.  The sponsoring organization is responsible for fueling the van throughout the duration of the trip and upon return.
  • Upon return, vehicles should be backed into one of the two designated spots in the last row of Chambers Hall upper-level parking lot.
  • Keys to University vans are to be picked up at Campus Police at the start time of your reservation. Keys cannot be picked up early, due to conflicts with other reservations.
  • Report any vehicle problems to Jeromy Yetter at jmy46@pitt.edu.
  • Report any accidents to the nearest police department or 911 in the event that the accident involves injury or irreparable damage to any vehicle involved. Contact Campus Police (724-836-9865) if the accident happened on/near campus.

Guidelines are subject to change.  Non-compliance with the above guidelines may result in disciplinary actions and may also jeopardize future usage of University vehicles by the individual.
